Online Report Request Form

Incident and accident reports that are requested online cannot be mailed. They must be picked up in person at the Flagler Beach Police Department open Monday - Friday from 8:00 a.m. - 5:00 p.m. Requesting a report online is a good idea and can save you substantial time. Here's why.

To request a record online, please complete the form below and click "Submit". A valid e-mail address is required. Background checks are not currently available.

You will receive an e-mail verifying that your request has been received, and you will be notified via e-mail when your report is ready for pick-up at the Flagler Beach Police Department Headquarters. You must bring photo identification and present it to the FBPD staff member. Your report will be waiting for you.

Reports not taken by FBPD personnel are not available through the FBPD. Contact the agency or municipality responsible for the report to obtain a copy. If you need assistance please e-mail the Records Division at records@fbpd.org.
